Get ready to e-file your income tax return

Did you receive Excluded Workers Fund (EWF) benefits in 2021?

The Department of Labor (DOL) mailed you federal Form 1099-G with:

  • the amount of the EWF benefits you received in Box 6, and
  • the amount of New York State withholding in Box 11.

You must report EWF benefits when you file your federal and state income tax returns.

If you are e-filing your return, your software should prompt you to enter all the information from your Form 1099-G, including Box 6. If it does not, contact your software company for assistance.

If you are filing a paper return, report the amount in Box 6 as other income on:

  • line 8Z of Schedule 1 of the federal Form 1040, U.S. Individual Income Tax Return, and
  • line 16 of New York State Form IT-201, Resident Income Tax Return.

Free software may be limited to basic returns and may not be available for self-employment income, capital gains or losses, or rental income. If you are not eligible for a free filing option for the software provider you choose, the software provider may charge you a fee. 

Review the checklist below to ensure that have everything you need before you begin; and watch the videos we offer to assist you. 

What you'll need

  • an active email account (enables you to keep track of the status of your return)
  • a copy of last year’s tax return or your prior year federal adjusted gross income (FAGI)
  • a driver license or state-issued non-driver ID (for you and your spouse, if filing jointly)
  • birth dates and Social Security numbers for you, your spouse, and your dependents
  • proof of income and tax withheld (wages, interest and dividend statements):
  • your tax account balance (the total amount you paid throughout the year toward your income tax)
  • Forms 1095-A, B, or C, Affordable Healthcare Statement
  • information related to any credits you’re claiming
  • your banking information (the account number and the routing number for your bank, which you can find on a blank check)
  • NY529 College Saving Account information (if requesting refund to be distributed)
